Case Management

Medical Care Case Management is available for all patients of the center or referrals from other agencies. These services include working with the HIV positive patient to obtain approval for the AIDS Drug Assistance Program (ADAP), housing, social security applications, home-makers, emergency funding and transportation to name just a few.

Correction Case Management is also available for ex-offenders recently discharged from a penal institution. The correction case manager is vital in assisting the ex-offender with obtaining medical care, medications, housing, substance abuse counseling, transportation, etc to make a smooth transition from the jail/prison to the community.
