Transportation

Transportation is a courtesy provided to clients who have no other means to get to appointments or services. Transportation is not an individual cab service, it is a shuttle van. Groups of people are picked up at intervals, thereby creating wait time.

Who can access transportation?

What is transportation used for?
  1. Primary care appointments
  2. Luck Care Center support groups (NA group, women’s group, adherence group, Monday night, PTL, etc.)
  3. Case management appointments
  4. Substance abuse counseling appointments
  5. Primary care related referrals

How to access transportation: For initial or follow-up primary care appointments, speak with the staff or receptionist to request transportation the same time you schedule your next office visit.

All requests for transportation must be approved by the physician or staff requesting the service at least 48 hours in advance. Your request for a return trip must be made the same time as the original pick-up request.

If you make a primary care appointment and discover the need for a ride later, you must call the office to request transportation at least 48 hours in advance of the scheduled appointment. Your request will be granted and confirmed based on priority and availability.

Confirmed appointments: If your appointment is scheduled between 9:00 am and 3:00 pm, please be ready for pick-up (2) hours in advance of your appointment time.  These hours are considered prime and other will be scheduled during the same frame. You will be called on the day of your appointment and given an estimated pick-up time.

If you do not have a telephone, you must be ready and waiting (2) hours before your appointment time.  It is acceptable to provide your neighbor’s phone number so we may inform you of the drivers’ estimated arrival time.

When weather, traffic and the cooperation of fellow passengers permit, you should arrive 15 -20 minutes before your appointment time.

Return Trip Home:  Please inform staff or receptionist to contact the transportation coordinator for your return trip home.

Please be patient, you will be picked up as soon as possible.  Waiting for a return trip may vary based on current whereabouts of the shuttle van.  If you have an alternate means of getting home and decide no to wait, inform a staff person so that the transportation request may be cancelled.

Cancellation: Please call the Luck Care Center to cancel any transportation request at (773) 233-5850. Please phone in advance, if possible.
